RN Learning - Transitions in Care: A Case Study

It is well appreciated that patients have better outcomes and higher satisfaction when care is effectively coordinated. The transition of care from an inpatient setting to the community often lacks the coordination of care needed to ensure the delivery of high-quality care that meets the needs of patients and families. This often leads to fragmented care resulting in re-hospitalizations, unnecessary care ,and excessive costs. The purpose of this nursing continuing professional development activity is to facilitate improvements in transitions of care from palliative inpatient settings to the community and to facilitate the development of processes to effectively manage patient care following acute care discharge.
